Utah State Facility Self-Inspection Survey

This application collects and stores data about the physical condition of over five thousand buildings owned by Utah state government.

Design Highlights:

• Based on the PHP MVC.
• JavasSript on the client side to make the application more interactive and user friendly.
• Reports are generated using PHP to create PDF documents which are compatible with most operating systems.
• Cascading Style Sheets make changes to the look and feel of the application easy to implement.
• Survey instructions, help files, cover letter and survey questions are stored in a MySql database and can be changed on they fly via a GUI interface.
